Medical Documentation for Clinics: Which Documents Does Your Practice Need?

A healthcare facility’s medical documentation includes records of patients and the care provided to them, maintained using forms approved by the Ministry of Health and in Ukraine’s electronic healthcare system. It also includes the internal documents used to organize the facility’s operations: orders, policies, standard operating procedures, informed consent forms, and agreements with patients.

Before opening a medical office, you need to establish the rules for working with patients. An operating clinic faces a different task: checking whether its documents reflect new services, personnel, and equipment. In both cases, we first determine how your particular practice operates and only then agree on the groups of documents required. You may order the full agreed package or a separate group. These legal documents for a private clinic or medical office are also known as medical-legal documentation, and developing medical documentation for a clinic always begins with how it actually operates.

Standard Operating Procedures (SOPs): Tailored to Your Practice

SOPs describe which staff members take action and in what sequence: identifying a patient, organizing an appointment, responding to an emergency, practicing hand hygiene, cleaning the premises, or reprocessing reusable medical devices. A treatment room, vaccination site, or dental practice needs different procedures from a consultation-only practice.

We select procedures based on your actual processes, premises, and equipment. For example, if a copied SOP refers to a sterilizer the clinic does not have or a position that does not exist on its staff, employees will not be able to follow it.

We handle the legal documentation and allocation of responsibilities; an appropriately qualified healthcare professional at your facility confirms the clinical accuracy of the medical sections.

Our case study, “How Many SOPs Does a Private Doctor Need: 9 Documents for a Gynecology Practice,” illustrates how many SOPs a small practice may need.

Documents for Working With Patients: Medical Services Agreement and Informed Consent

We prepare a medical services agreement or public offer, patient rules, documents for informed consent and refusal of medical interventions, and procedures for handling complaints and providing copies of medical records. For paid services, we agree on payment terms, appointment cancellations, and authorization of additional services.

The agreement sets out the service terms, while informed consent records the patient’s decision after the intervention has been explained. One document does not replace the other. Consent must correspond to the specific diagnostic, preventive, or treatment intervention; it cannot be reduced to a generic statement that the patient has no claims against the clinic. For each new procedure, we therefore check whether the current documentation covers it.

Internal Orders and Policies for Organizing Operations and Personnel

Internal orders bring documents into effect and appoint responsible persons. Policies and rules describe how appointments, internal quality control, complaints, departmental work, and document management are organized. If you have employees, we develop job descriptions and related HR documents within the agreed scope.

An individual entrepreneur running one medical office and a medical center with several departments need different document structures. We do not insert management bodies that do not exist into a small office’s documents. For a clinic, by contrast, we establish who approves documents, who trains staff to use them in their work, and who monitors compliance. If responsibilities exist only on paper, it can be difficult to determine who should have acted when an error occurs.

Primary Medical Records: Official Forms and Internal Recordkeeping

We identify the official primary medical record forms applicable to your practice and prepare internal rules for completing them, correcting errors, storing them, and providing copies. Within the agreed scope, we develop an internal document register, a patient inquiry log, an access register, and other recordkeeping forms and logs for specific processes.

An internal form does not replace state-approved medical records. If an official form applies to your activities—for example, outpatient medical record Form No. 025/o approved by Ministry of Health Order No. 110—we identify that form and the rules for using it. We also distinguish between paper records and work in the electronic healthcare system. An empty log without clear completion procedures does not prove that the recorded actions actually took place.

Personal Data and Medical Confidentiality

We prepare a personal data processing and protection policy, a patient privacy notice, rules for staff access and disclosure of information, and, where necessary, a data processing consent form. Consent is required for processing operations where consent itself is the legal basis; it cannot be treated as the sole legal basis for all processing of health data. We also assess whether patient photographs or case histories may be used in advertising.

This documentation is necessary for both a small medical office and a network of clinics. In a clinic, it must reflect the actual access granted to doctors, administrators, and other employees. A form signed by a patient does not solve the problem if that patient’s medical information is disclosed to an unauthorized person without a proper legal basis.

Documents for Specific Medical Processes

Depending on the services provided, we add documents covering infection prevention and control, medical waste management, medicine use, adverse reaction reporting, telemedicine, and care for children. What matters is what you actually do: prescribing a medicine during a consultation, administering it in the office, and maintaining a stock of it are three different activities.

We assess radiology, sedation, operating rooms, inpatient care, in-house laboratories, and activities involving controlled substances separately. Even one such process may require a custom scope of work. We do not include it in a standard package merely because the facility is small.

Which Scope of Work Is Right for Your Practice?

An individual-entrepreneur doctor with one medical office. If you provide services in a single consultation-based specialty, have up to three people in your practice, and do not perform invasive or specially regulated procedures, we begin with the Basic package. We identify the documents needed for an individual entrepreneur’s medical practice: patient appointments, patient relations, recordkeeping, and daily operations. You receive adapted files and instructions for putting them into effect. An audit of your existing document collection is not included in this package.

A dental office. We first determine which procedures are performed, how instruments are processed, and whether medicines, X-rays, or sedation are used. The Premium package may include one standard dental procedure module, excluding sedation and radiology. If either of those or other complex processes is involved, we prepare a customized proposal. The result is an agreed set of patient-facing and process documents, not a random collection of dental templates.

A clinic with several specialties. If a single entity operates at no more than two locations, has no more than ten employees, three departments, and three medical specialties, we consider the Premium package. It includes an initial audit within the specified limits and one standard procedure module. For a medium-sized facility, a network, or a more complex service mix, we first agree on a separate scope. The resulting documents connect departmental operations and allocate responsibility.

Examples of Our Work

The owner of a private dental office contacted us to organize the documentation and prepare for an inspection. We reviewed the existing documents and developed tailored SOPs covering the workflow from patient registration to archiving medical records. We specified the sequence of actions and responsible staff members so that the organization of work could be explained and verified. Read more about our work with the dental office.

When a multidisciplinary medical center was opening, we developed patient agreements, internal policies for staff, and related HR documents. These operating rules were prepared as part of the center’s launch, giving the client the documents needed to organize interactions with patients and employees. Read more about opening the medical center.

How We Work

To define the initial scope, we do not need named patient medical records, diagnoses, or test results. If examples of completed forms are needed later, we will agree on de-identification and a transfer method.

  1. We assess the scale of the practice. You provide information about the entity, locations, personnel, specialties, and services.
  2. We identify actual processes. We separate existing operations from planned specialties and procedures.
  3. We review existing documents. If an audit is part of the engagement, we identify what can be used, updated, or replaced.
  4. We agree on the document list, price, deadlines, and persons responsible for clarifications. Work outside the standard package is documented separately.
  5. We draft the texts and agree on them with you. Feedback is collected in agreed review rounds; the client’s relevant healthcare professional checks the clinical sections.
  6. We deliver the result. You receive editable files, a document list, an implementation guide, and the consultation included in your package.

Based on the agreed scope

For selective document orders, separate audits, complex procedures, practices outside the standard package limits, medium-sized facilities, or networks.

  • Assessing the practice and defining the specific task: individual standard operating procedures (SOPs), patient documents, an audit, an update, or a coordinated document set.
  • Agreeing on the number of entities, locations, departments, practice areas, and special processes.
  • Specifying the document list, audit scope, review rounds, and consultations in the commercial proposal and agreement.
  • Performing only the work expressly agreed; the procedure for adding new processes is determined before work on them begins.
  • Delivering the agreed result in editable format, with a document list and guidance on using the documents.

Result: documents or review findings for the specific task. For an audit without drafting new documents, we provide written comments and recommendations; for document development, we deliver the agreed files. You pay for a defined result, not for every category of document a healthcare facility could theoretically have.

Licensing, work in Ukraine’s electronic healthcare system, complex modules, training, and implementation are included only if expressly stated in the agreed scope. Without that agreement, they are excluded from the engagement.

Cost of Developing Medical Documentation: What Determines the Price?

The price depends on the size of the practice, the services actually provided, the number of locations and departments, the condition of existing documents, and the complexity of operations. Two clinics with the same number of employees may need different scopes of work: one provides consultations only, while the other performs procedures subject to additional safety and recordkeeping requirements.

The Basic package is intended for one consultation-based practice within specified limits. Premium adds an initial review of existing documents, several specialties, HR and process documentation, and one standard procedure module. We agree on a customized scope for individual groups of documents, in-depth audits, complex processes, medium-sized facilities, and networks. Prices and exact limits are set out in the service packages.

Before work begins, we specify which documents and consultations you will receive and how many review rounds are included. The number of files alone does not determine the price: their content, applicability, and consistency matter. If the practice expands or new processes are added after the scope has been agreed, we assess them separately.

Licensing, registration in Ukraine’s electronic healthcare system, setup of the medical information system, accreditation, contracts with the National Health Service of Ukraine (NHSU), team-wide training, and on-site implementation are not included in the standard packages. They may be ordered separately.

We agree on the final document list, price, and delivery period before work begins, once we receive the necessary information about the facility. Any new locations, specialties, or procedures introduced after approval are assessed separately.

The clinical sections of documents are approved by a competent healthcare professional at the facility, and the facility itself confirms that the documents reflect its actual processes. The facility is responsible for approving documents, familiarizing its staff with them, and maintaining the records. If needed, we can also help with these tasks under a separate agreement.

The package is a one-time engagement: ongoing legal support and subsequent document updates are not included.

Additional expenses—engaging an external specialist, site visits, and other outside costs—are incurred only under a separately approved budget. There is no government fee for preparing internal documents.

Why a Ready-Made Template Is Not Enough

The Ministry of Health, as the licensing authority, monitors compliance with medical practice requirements: it checks whether a facility complies with the Licensing Conditions for Conducting Medical Practice. Those conditions include maintaining Ministry of Health-approved primary medical record forms and using the electronic healthcare system. A practice’s internal documentation is therefore assessed for compliance with the licensing conditions.

A template can serve as a starting point if it has been reviewed and is appropriate for your activities. Problems arise when a document describes another facility: an SOP mentions equipment you do not have; an agreement uses different payment terms; or an order assigns responsibility to a position that does not exist. During an inspection, these inconsistencies can be compared with your actual operations, and in response to a patient complaint you will need to explain how the service was organized and documented. For example, generic consent “to all procedures” does not, by itself, show what was explained to the patient before a particular intervention. An empty log does not confirm that disinfection was carried out. These discrepancies must be considered when reviewing document content.

In the dental office engagement described above, we linked SOPs to the staff’s daily sequence of actions. This makes it clear who performs a procedure and where the result is recorded. Documents help explain and substantiate how work is organized, but they do not guarantee the absence of complaints, fines, or litigation.

What to Do After Receiving the Files

Documents must be approved through the procedure appropriate to your practice, communicated to staff, and used in daily operations. Staff complete medical records based on care actually provided. We do not backdate consents, orders, or records of actions that cannot be substantiated.

A documentation package does not replace a medical practice license, appropriate equipment, or staff qualifications. If we find that a document describes a process for which the organizational or licensing prerequisites are missing, we will flag the issue and distinguish document preparation from the work needed to remedy the discrepancy.

Answers to frequently asked questions

Which Documents Are Mandatory for an Individual Entrepreneur and a Clinic?

There is no single list that applies to everyone. The required set depends on the organizational form, specialties, services, staff, and equipment. Some documents demonstrate compliance with particular requirements; others structure operations and allocate responsibility. For your engagement, we identify the applicable list and do not promise “full compliance” based solely on the number of files delivered.

Can We Use Ready-Made SOPs Found Online?

They can be used as source material after their content has been reviewed and adapted. Check whether the equipment, materials, actions, and responsible persons match your actual practice. Changing the clinic’s name is not enough if the procedure itself is organized differently. The clinical workflow must be approved by a competent healthcare professional.

Is Informed Consent Required for Every Procedure?

Informed patient consent is required for the use of diagnostic, preventive, and treatment methods. This does not mean that each action automatically requires a separate new form: the way consent is documented depends on the intervention and the applicable forms. However, a general signature at the first visit should not be treated as consent to every future procedure. The law also provides an exception where there is an immediate threat to life and obtaining consent from the patient or legal representative is objectively impossible.

What Is Included in an Audit of Existing Documents?

Under the Premium package, we review up to 20 existing documents totaling no more than 100 pages, assessing their applicability, content, and consistency with operational processes and other documents. We identify what to retain, update, or replace. Reviewing more documents, conducting an on-site review of actual operations, or separately analyzing completed patient medical records requires additional agreement. An audit of existing documents is not included in the Basic package.

How Long Does Document Development Take?

We agree on the timeline after defining the scope and receiving the necessary materials. It depends on the number of processes, the condition of existing documents, and how quickly you respond and approve the clinical sections. Before work begins, we document the stages and delivery date. If you are preparing for a specific inspection or opening date, tell us at the outset so we can confirm which scope can realistically be completed.

How to Order Medical Documentation Development

The service may be ordered as a one-time engagement: we agree on and prepare a specific set of documents without requiring ongoing support. If you regularly deal with staffing issues, patient complaints, inspections, or operational changes, you may wish to consider ongoing legal services for medical institutions with an agreed scope of assistance.

To select a package, send us a list of your specialties, services, and locations, and briefly describe your task: opening, adding a new specialty, or reviewing existing documents. Based on this information, we will agree on the scope, price, and timeframe. Contact us through the form on our website or call us.
